Sports Physical Clinic
We are excited to be able to offer a Sports Physical clinic to students and athletes. Clinics are usually held at their school to make it convenient for the student and their families.

As a concierge Pediatrician, Dr. Ross believes in convenience and efficiency at an affordable cost.

We will be offering a Sports Physicals Clinic to the students of Albuquerque Academy on the following dates:
May 18 from 2pm to 6pm (fully booked)
May 19 from 2pm to 6pm (fully booked)
May 20 from 2pm to 6pm (fully booked)

Cost $25/student. You will receive a separate email with payment link.

Parents must complete the first 2 pages of the Sports physical form provided to you in your school's account. Our Pediatrician will complete the third page after carefully reviewing your responses and performing a physical exam.

Medication Authorization form (to be completed by parent)

If your child requires an updated Asthma Action Plan or updated Allergy Action plan, a parent must be present or easily accessible via phone to answer any questions.
​
Completed forms will be returned to each student immediately after your appointment to upload to your Magnus Student Health record.
​
Immunizations will not be offered at this clinic. Recommend making an appointment with your Pediatrician if your child needs to be up to date with vaccines.
