About the event
Face to Face Pediatrics is happy to be able to provide on site sports physicals for the students of Albuquerque Academy. We hope that parents will appreciate this added convenience from a Board Certified Pediatrician, Dr. Ross. All students are required to be present with a completed and signed Sports Physical form (obtained from your Magnus Health Record or on our website) by a parent. If a student requires an Allergy or Asthma Action plan form to be completed, a parent must be present or at least easily accessible by phone during the time of the visit for questions. All forms are required to be uploaded to your Magnus Health Account.
