2023 Sports Physical Clinic
We are excited to be able to offer a Sports Physical clinic to students and athletes. Clinics are usually held at their school to make it convenient for the student and their families.

We are excited to offer a Sports Physicals Clinic to the students of Albuquerque Academy on the following dates:
Monday 4/24/2023 @ 9am to 4pm (no school at Albuquerque Academy)
Tuesday 4/25/2023 @ 3pm to 7pm
Wednesday 4/26/2023 @ 3pm to 7pm
Thursday 4/27/2023 @ 3pm to 7pm

Cost $40/student

Parents must complete the first two(2) pages of the Sports physical form provided to you in your school's account. Our Pediatrician will complete the third page after carefully reviewing your responses and performing a physical exam.

Medication Authorization form (to be completed by parent)

If your child requires an updated Asthma Action Plan or updated Allergy Action plan, a parent must be present or easily accessible via phone to answer any questions.
​
Completed forms will be returned to each student immediately after your appointment to upload to your Magnus Student Health record (school requirement.)
​
Immunizations will not be offered at this clinic. Recommend making an appointment with your Pediatrician if your child needs to be up to date with vaccines.
